| Players Registered with CYSA in the previous season (2009) will receive a preprinted registration form that must be completed and signed. |
|
|
|
|
|
|
|
|
|
|
|
|
| Any NEW registrant or persons without a preprinted form will be required to complete a registration form when they come to the office to register. |
| All new registrants must present proof of age. |
|
|
|
|
|

Late Registration Policy -
Final date for registration will be Monday February 1st, 2010; players interested after this date will need to call |
| the office at 519-352-1999 and leave a message stating NAME, GENDER, YEAR OF BIRTH, & PHONE NUMBER. |
|
|
| These individuals may then be contacted should there become an opening in his/her age group. |
|
|
|

|
Refund Policy -
To compensate for costs associated with player registration all refunds wil be subject to an Administration Fee of; |
|
|
|
|
|
|
|
|
|
|
|
|
|
|
1) $20 if request is received prior to player being selected/drafted to a team. |
|
|
|
|
|
2) 50% of player registration cost if drafted to a team and prior to July 1st, 2010.
|
|
|
|
|
| Only requests for refunds received at the soccer office in writing or in person and accompanied by the current team jersey will be processed. |
|
|
|
|
|
|
|
|
|
|
|
|
| NO REFUNDS WILL BE PROCESSED AFTER JUNE 30 |
